What to expect:
Prior to the visit, an intake form will be sent to obtain an overall history. Assessment, support, education, counseling. Individualized plan. Unlimited phone and email support for 1 week after the visit. Resources will be email after visit as needed.
Payment information
I am an Out-Of-Network provider. Payment via cash, check, venmo, paypal is due at time of scheduled appointment.
All lab work will be billed directly to your insurance
Families in need, please reach out regarding sliding scale. Link to zoom and payment info will be emailed to you.
